Enhanced Discharge Planning: Patient Rights, Advocacy and ResourcesPresenter: Paula Wolfson, LCSWPaula Wolfson, LCSW, Avenidas care consultant, will provide you with timely guidelines for navigating skilled inpatient care settings (ER, hospital, rehab facilities, hospice care) and for optimizing discharge to assisted living and memory care, home and home health care, adult day health care, or palliative care. Handouts will include tips for calling 911 and Adult Protective Services, accessing dementia care resources, fall prevention, and designing contingency plans for caregivers.This is an information packed session.
